Family-Focused Therapy
(FFT) for Bipolar Disorder

Empowering Wellness Through Knowledge

Family-Focused Therapy (FFT) is an evidence-based treatment designed specifically for individuals living with bipolar disorder. Originally developed by Dr. David Miklowitz and colleagues, FFT combines psychoeducation, communication skills training, and problem-solving strategies to help reduce mood episodes, improve relationships, and enhance long-term stability.

FFT helps individuals and their loved ones better understand bipolar disorder, recognize early warning signs of mood changes, navigate conflict more effectively, and build a supportive environment that promotes recovery. Research has shown that FFT can reduce relapse rates, improve functioning, and strengthen family relationships when used alongside appropriate psychiatric care.

The standard FFT model consists of approximately 21 sessions delivered over 9 months. Sessions may involve partners, family members, or other significant support persons and are structured to provide education about bipolar disorder, improve communication patterns, and develop effective problem-solving skills. Individualized adaptations are also available for those who wish to apply FFT principles without direct family participation.

Whether you are newly diagnosed or have been managing bipolar disorder for years, FFT can provide practical tools to help you build resilience, improve communication, strengthen your support system, and maintain long-term wellness.

Katrin is one of only a handful of clinicians in the United States with specialized training in FFT for Bipolar Disorder. Drawing on this evidence-based approach, she helps individuals and families better understand the disorder, navigate challenges together, and create a supportive environment that promotes lasting recovery and well-being.

How is Family-Focused Therapy Used?

FFT is used to help individuals with bipolar disorder and their loved ones better understand the illness, improve communication, and develop practical strategies for managing symptoms. Through structured sessions, therapists provide education about bipolar disorder, teach effective communication skills, and guide families in solving problems that may contribute to stress or conflict. FFT helps family members recognize early warning signs of mood episodes, respond supportively during difficult periods, and create a more stable home environment. By strengthening relationships and building a collaborative support system, FFT can improve treatment outcomes and reduce the risk of relapse.

Why Family-Focused Therapy Works

Reducing Stigma Through Education

Bipolar disorder is often misunderstood, leading to fear, stigma, and misconceptions that can affect both individuals and their families. A core component of FFT is psychoeducation, which helps families gain an accurate understanding of bipolar disorder, including its symptoms, causes, treatment options, and impact on daily life. By replacing myths with knowledge, FFT helps reduce anxiety, foster empathy, and create a supportive environment where family members can openly discuss their experiences and concerns.

Meeting the Needs of All Family Members Through Collaborative Care

Bipolar disorder affects more than just the person diagnosed—it can influence family relationships, communication patterns, and overall household functioning. FFT recognizes that every family member may need support. Through guided discussions, family members are encouraged to express their thoughts, emotions, and needs in a safe, nonjudgmental setting. This collaborative approach helps families heal strained relationships, strengthen emotional connections, and work together toward shared goals.

As FFT progresses, families often develop a stronger alliance. They begin to understand how factors such as stress, functional impairments, biological predispositions, and personal responsibility influence the family's overall functioning. Through a collaborative care approach, family members learn self-management strategies to cope with their emotions and the patient's symptoms. Each member gains a voice in the treatment planning process, contributing to the identification of family goals.

Preventing Relapse

During the psychoeducation phase, families learn about bipolar disorder and family dynamics and are introduced to relapse prevention planning. They identify contexts that have previously increased the patient's risk of relapse, such as substance use, lack of structure, or changes in eating/sleeping habits. The clinician then assists the family in developing concrete steps to help the patient return to a healthy path or seek additional support when needed.

Building a Stronger Family Partnership

As therapy progresses, families develop a deeper understanding of how stress, biological factors, life circumstances, and personal choices can affect mood stability and family well-being. FFT empowers each family member to play an active role in treatment planning while learning practical strategies to manage stress, support recovery, and promote long-term wellness.

Enhancing Communication Skills

Healthy communication is essential for maintaining supportive relationships. FFT teaches family members how to express themselves clearly, listen effectively, and respond constructively during difficult conversations. Through coaching, role-playing, and skill-building exercises, families learn communication techniques that reduce conflict, increase understanding, and strengthen emotional support.

Strengthening Problem-Solving Skills

Families often face challenges related to managing symptoms, navigating stressors, and balancing daily responsibilities. FFT provides structured problem-solving tools that help family members work together to identify challenges, evaluate solutions, and make decisions collaboratively. These skills can improve family functioning and increase confidence when addressing future obstacles.

Creating Lasting Change

Living with bipolar disorder can present challenges for both individuals and their loved ones. Family-Focused Therapy offers a structured, evidence-based approach that promotes understanding, resilience, and stronger relationships. By fostering education, communication, and collaboration, FFT helps families become a powerful source of support in the journey toward long-term stability and recovery.
